To give through payroll deductions at work through the United Way:

Select OTHER CHARITY and under Charity Name put:

Elks National Foundation
2750 North Lakeview Ave
Chicago, Il 60614

As long as you indicate to the United Way on your pledge information that they're able to release your information to the charity (the Elks National Foundation), they will be able to credit the donation to your donor record when theyreceive it, and in turn, credit our Lodge's per capita.

Sometimes United Way sends funds from "anonymous" donors, but it's your choice to remain anonymous or not. So just be sure to permit the United way to release your donor information, and you shouldn't have to do anything else!
(I just put a statement saying "Please release my name and information to Elks National Foundation" and I initial it, to be sure.

Also note:
Payroll deductions often take several months to reach the ENF office. It goes from your payroll department to be processed by the United Way, then United Way sends a disbursement check to ENF either monthly, quarterly or annually. Quarterly and annually are more common. So although your donations have been deducted already from your paycheck, it may take awhile for ENF to recieve and post it to your account.
